Why most technology projects underdeliver — and what changes the outcome

The system that works is not always the system that was specified. Experience is the difference between a design that survives contact with reality and one that does not.

The failure patterns in technology programmes are well established. Scope creep, underestimated integration complexity, poor requirements definition, inadequate testing environments and weak change management account for the majority of project overruns. These are structural problems, not technical ones, and they require structural responses.
